From within the Activities > Add Document screen;

Select Document

You can either drag and drop a document into the box or manually browser to find the document.

Releated Object Type

Select the Object Type the document is

Related Object Reference

Start typing the name of the object that the document belongs to.

Document Structure

Start typing the name of the structure that the document is to be attached to

Document Type

Select the type of document that you are attaching.

Status

Choose whether the document is a Draft or Final copy.

Instance Name

Site

Select the site where the document is to be stored.

Internal Reference

Enter an internal reference for the document.

Tags

Add any required tags that you may want to search for this document with.

Notes

Enter any notes that you want to attach to the document
