Creating a HSEQ Observation Induction_

When doing HSEQ (Health and Safety Environment Quality you can define sets of questions that you want to ask of the staff who are present on site. This process uses Inductions to define the questions. The major difference between a standard induction and an induction used for a HSEQ observation is that you do not have a single correct answer as it is what the person performing it observes. To facilitate this there is a new option Allow Multiple Correct Answers?


Firstly you need to create a new induction. In the first screen, there is a new option  Allow Multiple Correct Answers? . This must be ticked to use the induction for HSEQ observations.



You then add a group for the questions;

  1. Enter a group name.
  2. Click on the tick to add it.

You will then see the group within the accordion.

When you click on the group link the group screen is displayed and you can then add the items.

You can then add the questions;

  1. Enter a title for the question.
  2. Click on the tick.


When the Item  Name link is displayed click on it to add the question.



You will then see the Add Question screen.


When you click on the Add Question button the following screen is displayed;

  1. Enter the question.
  2. Enter a response.
  3. Tick the Correct box.
  4. When you have entered the valid responses (All must be set to correct) click on Save.


You will then see the Question and answers


To add another question click on the breadcrumb for the question group;



Then repeat the process of adding a question.


When you have added all of your questions



  1. Click on the breadcrumb to take you back to the induction.
  2. Click on Publish Draft.
  3. Click on Publish Draft.
