Adding a Response Template

  1. Select the answer type.

  2. Give it a meaningful name that clearly states what the responses are.

  3. Give it a meaningful description.

  4. Determine whether they have to make a response and whether they can make multiple responses. 0 Indicates that answers are optional.

  5. Click Save.

Adding Responses to the template

When you have added the template you can define the responses that go with it.

  1. Click on Add Response.

  1. Add the text for the response.

  2. Add the label for the response.

  3. If you want to allow the customer to add some additional information place a tick in the check box.

Repeat as required.
